Children who meet the following criteria are invited to join PLAY/LFBK After School Program:
- At least 4 years of age before his/her first day of a current school semester
- Potty trained
- Comfortable with separation
- Communicates in English

School Pick Up (by Play Kids/LFBK Staff)
- Children enrolled in after school program with a school pick up option will be picked up from school and escorted to Play Kids Greenpoint facility by our Staff.
- Parents must complete registration forms and provide school details (dismissal time, grade/classroom, teacher’s name). Play Greenpoint must be added to authorized pick-up persons.

 

After School Pick Up
- Parent / caregiver must sign out daily at pick-up.
- For security purposes, PLAY / LFBK must be notified 24 hours in advance if there will be an alternate person approved to pick-up.
- The day dismisses at 6pm. Caregivers should arrive 10 minutes prior. PLAY / LFBK reserves the right to charge a late fee of $15 per 15 minutes after scheduled dismissal time.

Cancellation and Refunds
- A payment in full or installment plan is required to be guaranteed a spot.

- A 25% cancellation fee applies to all reservations.

- For cancellations made between 30 and 15 days prior to the after school start date, you will receive a 50% refund.
- For cancellations 2 weeks or less from the first day of after school, there will be no refunds made. However, for 
cancellations due to medical emergency, a 50% of the total amount can be applied to other products & services at Play Kids Greenpoint for up to 6 months.
- A payment in an installment plan is considered as enrollment for the full semester. Cancellations/withdrawing from the program while the semester is in session do not cancel the installment plan. All installments will be charged as scheduled upon registration.
- Refund conditions are based upon enrollment, not attendance. Refunds or a proration cannot be granted for absences due to illness or vacation.
- Refunds will be considered in the case of overpayments or fees paid after the cancellation notice (please allow two weeks). There will be no refund in the event that your child is suspended or discharged from the program.

- For cancellations due to Covid-19 a positive PCR test is required (sent by email). Cancellation due to Covid-19 will be a subject for a credit only. Maximum days cancelled can’t be more than an estimated quarantine time. No refunds. Credit is non-refundable and non-transferable.
- 50% of refund is returned to original form of payment, remaining percentage is issued in the form of PLAY / LFBK credit. Credit valid for up to 3 months for PLAY services. There will be no refunds made for credits. 
- Any refund is by check. 4% credit card fee billed by merchant bank to PLAY / LFBK will be charged on all refunds.

Make-ups
There will be no refunds provided for missed days of program. Every registration includes 2 make up class options per semester. Make-ups are subject to availability.

Food, Allergies and Outside Items
PLAY /LFBK requests that all members, their children, caregivers and guests be forewarned that NO ALLERGY-RISK FOODS CONTAINING NUTS, nut ingredients or derivatives, be brought into our premises. We utilize our best efforts to make PLAY /LFBK safe for children with food allergies, and do not knowingly serve food that includes peanut or nut products.

While we are “nut aware”, we cannot guarantee that we will be “nut free” at all times. PLAY /LFBK requests that all food allergies be listed in the Medical Information portion of this form.
